What is a GPCCMP?
A GP Chronic Condition Management Plan (GPCCMP) is a structured plan developed by your General Practitioner (GP) for patients with a chronic or complex health condition that has lasted, or is expected to last, six months or longer. It:

Outlines the allied health services you can access
Provides action steps to help you manage your condition effectively

Getting Started
Consult your GP – Discuss your chronic condition and the potential benefits of a GP Management Plan (GPMP)
Obtain a referral - Your GP will provide a referral to our clinic. Ensure it is addressed to Macquarie Health Hub (or your chosen practitioner) and clearly states the number of sessions approved.
Submit your referral - Ask your GP to send your referral directly to Macquarie Health Hub, or bring a copy with you to your first appointment.
Book your appointment - Contact our team to schedule your first session.
Note: Referrals are valid for 18 months from the first service date.
